One-off $10.00 fee to cover printing and accessory costs.
How to order & what to expect
Step 1: Submit your details
Fill in our online request form below with your diagnosis, emergency contact details, and choice of card format. If requesting the Plastic Photo ID option, you can upload a clear photo of yourself.
Step 2: Verification & payment
Our team verifies your details. If you selected the $10 Plastic Photo ID option, we will provide simple payment instructions via online bank transfer.
Step 3: Fast delivery
Your completed card, protective sleeve, or lanyard will be posted directly to your home address within 5–7 business days.
